5. Cookies & Tracking Technologies

This Site's contact and quote-request forms are not intended to collect protected health information (PHI), and we ask that you not submit detailed medical history or health records through the Site. Price Services Group, L.L.C. is a licensed insurance agency, not a HIPAA-covered entity or a healthcare provider, and this Site is not a substitute for communication with your doctor or health plan. If you share health-related information with us during a phone call or in-person consultation in connection with an insurance application, we handle that information confidentially and use it only as necessary to help you apply for or compare coverage.

The Site is not directed to children under the age of 18, and we do not knowingly collect personal information from children.
